CASE #5: 27 year old man patient with right "near" dehiscence of the superior canal arcuate eminence (blue-lined superior canal) and dural herniation through skull base (tegmen) defect. He presented with intolerable aural (ear) fullness, autophony, hyperacusis (sound sensitivity) and chronic fatigue. Surgery was performed using a small incision and mini-craniotomy technique. A, thin (blue-lined or near dehiscence) of the superior canal arcuate eminence. B, 1.5 millimeter diamond burr used to thin bone to endosteum of superior canal. C, 2 millimeter ball of bone wax used to gently repair the defect. D, stable repair of defect with bone wax shown.
